RESUMO

Objective:To establish a method of ion chromatography for the determination of sodium, potassium and citric acid in potassium sodium hydrogen citrate granule. Methods:The chromatographic conditions for potassium and sodium were as follows:a Di-onex IonPac CS12A column (250 mm × 4. 6 mm, 5μm), 0. 02 mol·L-1 methane sulfonic acid solution as the mobile phase, the flow rate of 1. 0 ml·min-1 , the suppressor of CSRS 300, the inhibition current of 59 mA, the inhibition type conductivity detector, and the injection volume of 25 μl. The chromatographic conditions for citrate were as follows: a Dionex HPICE-AS1 colunm ( 250 mm × 9. 0 mm,7. 5 μm), 0. 015 mol·L-1sulfuric acid solution as the mobile phase, the flow rate of 0. 6 ml·min-1, the detection wave-length of 220 nm and the injection volume of 10 μl. Results: The linear range of sodium was 0.82-82.49 μg·ml-1(r=0.9999), and the average recovery was 98.9%(RSD =0.55%, n =9). The linear range of potassium was 1.38-137.89 μg·ml-1 (r =1. 0000), and the average recovery was 100. 5% (RSD=0. 53%, n=9). The linear range of citric acid was 0. 021-10. 600 μg· ml-1(r=1. 0000), and the average recovery was 99. 1% (RSD=0. 54%, n=9). Conclusion:The method is simple, rapid and ac-curate, and can be used for the quality control of potassium sodium hydrogen citrate granule.

RESUMO

<p><b>OBJECTIVE</b>To study the clinicopathologic features, immunophenotype and prognosis of matrix-producing metaplastic carcinoma (MPC).</p><p><b>METHODS</b>Sixteen cases of MPC diagnosed between 2002 and 2012 in West China Hospital were identified. The clinicopathologic features were analyzed. Immunohistochemistry for E-cadherin, S-100 protein, CK5/6, HCK, PCK, CK7, CK8, p63, SMA, EMA, CD99, MSA, CK14, EGFR, ER, PR, HER2 and Ki-67 was performed with EnVision method. The clinical outcome was evaluated and compared to matched controls of invasive ductal carcinoma.</p><p><b>RESULTS</b>All patients were women and ranged in age from 29 to 69 years (median age 48 years). The median size of primary tumor was 4 cm. Most of the tumors were well-circumscribed with expansile and multinodular appearance. Histology showed invasive carcinoma with a direct transition from carcinoma to cartilaginous/chondromyxoid matrix without an intervening spindle cell component. Tumor distribution was either nodular or diffuse. The matrix component accounted for 10%-80% of the tumor volume. All the tumors were strongly positive for S-100 protein and basal-like cytokeratin with triple negative phenotype (ER, PR and HER2 negative). Alcian blue stain was positive for the cartilaginous/chondromyxoid matrix. Compared with invasive ductal carcinoma, patients with MPC had increased locoregional recurrence (P = 0.010), increased distant recurrence (P = 0.011) and shorter disease-free survival (P = 0.017).</p><p><b>CONCLUSIONS</b>MPC is a rare variant of mammary metaplastic carcinoma with unique characteristics of morphology and immunohistochemical staining pattern. This subtype seems to have aggressive biologic behavior.</p>

RESUMO

Objective:To observe the clinical effect of piribedil and madopar on Parkinson's disease ( PD) . Methods:Totally 58 patients with PD were divided randomly into the treatment group (29 cases) and the control group (29 cases). Piribedil and madopar were used in the treatment group, while only madopar was applied in the control group. Therapeutic effect of both groups was evaluated by UPDRS at the end of 6 courses. Results:The decrease of UPDRS after the remedy in the treatment group was more significant than that in the control group (P<0. 05), and the total effective rate in the treatment group(89. 7%) was significantly higher than that in the control group(65. 5%)(P<0. 05). Conclusion:The clinical effect of piribedil combined with madopar on PD is superior to that of madopar alone, therefore, the combination use is a useful method in the treatment of PD with the value of popularization and applica-tion.

RESUMO

Purpose To study the clinicopathologic and genetic features of clear cell sarcoma of the gastrointestinal tract. Methods One case of clear cell sarcoma of the gastrointestinal tract was analyzed by histology, immunohistochemistry, fluorescence in situ hy-bridization ( FISH) , with review of relevant literatures. Results A female patient was admitted to the hospital with a history of 1-week abdominal pain. Computed tomography revealed a mass in right hemicolon. A grey-white ulcerated mass was observed in the colon, in-volving the entire thickness of the colon wall extending into the subserosa. Microscopically, medium-size oval or round cells were ar-ranged in sheets and scattered osteoclast-like multinucleated giant cells were also presented. Immunohistochemically, the tumor cells were strongly positive for S-100 protein, but negative for HMB-45, Melan-A, CD117, CD1a and PCK. FISH showed the rearrangement of EWSR1 in 74% of the neoplasm cells. Conclusions Clear cell sarcoma of the gastrointestinal tract is an extraordinarily rare gastro-intestinal neoplasm that shows peculiar histopathological, immunohistochemical, ultrastructural, and genetic features. Whether clear cell sarcoma-like gastrointestinal tumor represents a distinct entity awaits larger series, including cytogenetic or molecular biological in-vestigation.

OBJECTIVE@#To explore the role of IL-21 in the pathogenesis of myasthenia gravis (MG) and its influence on the the class switch of anti-AChR antibodies.@*METHODS@#Blood was taken from 26 patients and 18 healthy controls, and the expression of IL-21R mRNA in peripheral blood mononuclear cells (PBMCs) was detected by RT-PCR. The expression of IL-21R on B lymphocytes was measured by flow cytometry, while the concentrations of serum IL-21 and the levels of anti-AChR-IgG and its isotype IgG(1), IgG(2), and IgG(3) were tested by ELISA.@*RESULTS@#The serum concentration of IL-21 in the MG group was higher than that in the control group (31.686±8.499 pg/mL, 15.147±6.366 pg/mL) and the difference was significant (P0.05). Compared with the control group, the expression of IL-21R on B lymphocytes also increased in the MG group (P0.05). Expression of IL-21R mRNA in the PBMCs showed no correlation with the level of serum anti-AChR-IgG and its isotype IgG(1), IgG(2), and IgG(3), respectively(P>0.05); however the expression of IL-21R in B lymphocytes showed positive correlation with anti-AChR-IgG and it's isotype IgG(1) and IgG(3) (P0.05).@*CONCLUSION@#IL-21 might induce the class switch of anti-AChR antibodies to IgG(1) and IgG(3) isotype through IL-21R on B lymphocytes which promotes the pathogenesis of the MG.
